Dear ABE-LA Teachers,

The form will take approximately 25 minutes to complete.

PLEASE NOTE: The data generated from this form will be copied into a spreadsheet at the end of the year which will be reported to our national office. Our grant funding is determined by this report. Please be sure to read the questions and examples thoroughly before answering. Follow all instructions including using Arabic numerals (e.g., 1, 33, 502) and NOT using abbreviations. For example, if asked for your total number of classes, you would enter "3" NOT "3 Bio Classes".  

You may order any of these lab sequences:
* COMPLETE GENETIC ENGINEERING SEQUENCE (Labs 1, 2, 3, 4, 5) with optional Lab 6 and colony PCR
* ABRIDGED GENETIC ENGINEERING SEQUENCE (Labs 1, 2A, 4A, 5A) with optional Lab 6 and colony PCR
* FOCUS ON BACTERIA SEQUENCE (Labs 1 and 5B) with optional Lab 6 Protein Purification
* INTRO TO BIOTECH SEQUENCE (Lab 1 only — pipetting and gel electrophoresis, and optional extensions)

EACH TEACHER must place their own order at least ONE MONTH before their reservation date and submit a signed ABE-LA Teachers Expectations and Policies form. If you are using materials on two different dates (Fall and Spring term), then you should complete a new supply order each time. If you are unsure of what numbers to provide to us at this time, just enter your anticipated numbers for students, groups, and classes so that we can begin to allocate accordingly. Then confirm your actual numbers about 3 WEEKS AHEAD. We look forward to a new school year with you!
