2023 Healing Place Charity Classic Golf  Tournament Registration Form October 2, 2023
The 2023 Healing Place Charity Classic Golf Tournament will take place on Monday, October 2nd 2023 at Cypress Lakes in Muscle Shoals, AL. There will be morning and afternoon start times at 8am and 1pm and each start time will be limited to 18 teams per start time. Registration fee includes team registration, players breakfast, players lunch, and 2 mulligans. The registration fee can be paid in the following ways:
-Mail check or money order to The Healing Place PO Box 2765, Muscle Shoals, AL 35662 (checks should be made payable to The Healing Place)
-Call The Healing Place at 256-383-7133 to pay over the phone via credit card
-Pay in person at the tournament
-Email lmauldon@thehealingplaceinfo.org to arrange a time for a healing place staff member to collect payment from your place of business

Payment/registration deadline is September 30, 2023 unless you choose to pay in person at the tournament
