1. Independent Sales Representative is fluent in English and does not require any translations of this form, the IC Privacy Policy, and any forthcoming independent contractor agreement or U.S. tax forms.

2. This registration does not give rise to any legal obligations on behalf of any party, does not create any contractual relationship between the Independent Sales Representative and ATTCON International Corporation, and that any contractual relationship can only be formed by executing an Independent Sales Representative agreement.
