By completing this form, you are saying you would like for your child to participate in SanLee Middle School's SGLA Summer Bridge Program, which will be held July 31 to August 3 from 8:00 am to 12:00 pm each day.  

To register your child, please submit this form by July 7, 2023.  (If the form is submitted after July 7, 2023, there is no guarantee that your child will be able to attend academy nor receive a shirt).
