DPS Student Data Privacy - Parent Consent Letter - Asbury Elementary
NOTE:  Please fill this out for each of your students at Asbury.  When you submit the first one, you can click to submit an additional one.  Thank you for your time.

Dear Parent(s),

This School and its teachers use several online tools to engage students and drive their learning. On our District’s Academic Technology Menu (ATM): http://atm.dpsk12.org/usage_reporting.aspx is a list of online tools that one or more of your student's teacher(s) would like to utilize as an online learning tool. Before using any of the tools, we wanted to make you aware of federal laws and regulations that apply to the use of technology to support classrooms, and require parent consent in order to be most effective.

FERPA

In order for a teacher to use an online tool with student records or other information that could be personally identifiable, the Family Education Rights and Privacy Act (FERPA) requires that we obtain your consent to disclose your student's information to the website's operator or to other participants for the limited purpose of accessing the online tool.

COPPA

In order for students to use these programs/services, certain personal information must be provided to the web site operator. Under the Children's Online Privacy Protection Act (COPPA), these websites must provide parental notification and obtain parental consent before collecting personal information from children under the age of 13. For more information on COPPA, please visit https://www.ftc.gov/tips-advice/business-center/privacy-and-security/children%27s-privacy.

Please take a moment to review the ATM to see the new non-FERPA approved online tools we will be using at our School. (Visit http://atm.dpsk12.org/usage_reporting.aspx and then select your student’s School under the “School Breakdowns” section). Please visit any of the websites to see what personal information the site collects. When an email address is needed, we will have your student use his or her District-provided email account when using these sites.

After reviewing these sites, please sign this form by 8/31/2020 below using the e-signature format (e.g., /John T. Smith/). This will serve as your permission for your student to have access to the sites listed at http://atm.dpsk12.org/usage_reporting.aspx for our School, as well as your permission for us to upload your student's information into the application so that we can make the most of its functions.

At this time, consent is applicable to all non-FERPA approved online tools.  Thus,  if you choose not to sign this permission form, your student will not participate in any of the online tools listed. This permission will expire at the end of this school year, and can be revoked any time.

The school or your student's teacher may add new non-FERPA approved online tools to this list during the reminder of the year. When new online tools are added, parents will be notified by the school.

Sincerely,

Jeanna Doung - Asbury Principal
