This form allows you to choose your team's "favorite" or "best" entry from the official phase of the PhysioNet/CinC Challenge 2020.

Your favorite entry does not need to be your most recent entry or your highest scoring entry. It can be any successful entry from the official phase of the Challenge.

Please complete this form no later than 11:59pm GMT on 2 September 2020. If you do not complete this form, then we will evaluate your highest-scoring entry from the official phase of the Challenge.

Since the best entry is one of your previous entries, it does not count as one of your 10 allowed entries.
