Quest Academy follows strict rules to protect the privacy and safety of its students.  Where required by law, the school obtains the written consent of a parent prior to disclosing his/her student’s data outside of the school.

Occasionally at our school, teachers in the process of completing their licensure or obtaining additional licenses or endorsements are required by their colleges/universities to record their classroom sessions.  Such recordings are then shared with the teacher’s program officials for evaluation purposes. Our teachers have been instructed to not share unnecessary student data during the recordings and to call on students by using only their first names, so as to not share additional personally identifiable information.

Your student’s class may be the subject of such a classroom recording during the school year.  Therefore, we request that you please complete this form and return it to your student’s teacher by Friday, December 11th.  If you elect to not have your student attend class during such a recording, he/she will be given an alternate assignment in a different location in the school for that class period.
