Please complete this form to pre-register your child for the MLC Summer Program 2021. Each child will need their own pre-registration form so please fill out a form for each child that will be attending.
Filling this form out does NOT hold a spot for your child in our program. Your child's spot is not guaranteed until all registration paperwork is complete and the deposit is paid. This is step one in the process.

The camp operates Monday through Friday from 7:30am to 4:30pm from June to September. The MLC welcomes children of all abilities, and those with special needs are given individual attention by trained staff. Breakfast, lunch, and snack are served.  Parent drop off and pick up is required.
