INFORMATION CLAUSE

In connection with the use of support in the project "Integration, adaptation, acceptance. Support for third-country nationals living in Lower Silesia - Stage II" co-financed from the Asylum, Migration and Integration Fund and the state budget, I acknowledge that:
1. The administrator of my data is Fundacja Ukraina (Partner - project implementer) with its registered office at ul. Ruska 46a/201, 50-079 Wrocław, tel. 571 330 203, e-mail: biuro@fundacjaukraina.eu
2. Any questions regarding personal data can be sent to the e-mail address: iod@fundacjaukraina.eu, the registered office address indicated in par. 1) or phone number 571 330 203.

3. My personal data is/will be processed in order to implement the project, in particular to confirm the eligibility of expenditure, provide support, monitoring, evaluation, control, audit and reporting as well as information and promotion activities, as well as for purposes related to the recovery of funds, archival purposes and statistical;
4. The processing of my personal data is lawful and meets the conditions referred to in art. 6 sec. 1 lit. b and c and art. 9 sec. 2 lit. and Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation ) (OJ L 119 of 04/05/2016, p. 1), hereinafter referred to as GDPR.
5. The recipients of my personal data will be:
Partner - project implementer,
Beneficiary - Project leader, i.e. Lower Silesian Voivodeship Office with its registered office at ul. Plac Powstańców Warszawy 1, 50-153 Wrocław,
European Projects Service Center of the Ministry of the Interior and Administration with its registered office at ul. Puławska 99a, 02-595 Warsaw
The Ministry of the Interior and Administration with its registered office at ul. Stefana Batory 5, 02-591 Warsaw and entities that, at the request of the Partner - project implementer and the Beneficiary, participate in the implementation of the project. Personal data may be transferred for the purpose of monitoring, reporting and auditing the implemented project only to entities authorized to carry out the above activities
6. My personal data will be stored for a period of 6 years from the approval by the Delegated Body (i.e. the Center for European Projects of the Ministry of Interior and Administration) of the final report on the implementation of the Project;
7. Providing data is a prerequisite for receiving support, and refusal to provide them is tantamount to the inability to provide support under the project.

8. I have the right to access my data and the right to rectify it, limit processing, raise objections. In this case, the right to delete data does not apply, because the processing of my data is necessary to comply with a legal obligation requiring processing under European Union law and the law of the Member State to which the Administrator is subject.
9. I have the right to lodge a complaint with the President of the Data Protection Office when I consider that the processing of personal data violates the provisions of the GDPR.

KLAUZULA INFORMACYJNA

W związku z korzystaniem ze wsparcia w projekcie „Integracja, adaptacja, akceptacja. Wsparcie obywateli państw trzecich zamieszkałych na Dolnym Śląsku – Etap II” współfinansowanego z Funduszu Azylu, Migracji i Integracji oraz budżetu państwa przyjmuję do wiadomości, iż:
1. Administratorem moich danych jest Fundacja Ukraina (Partner - realizator projektu) z siedzibą przy ul. Ruskiej 46a/201, 50-079 Wrocław, tel. 571 330 203 e-mail: biuro@fundacjaukraina.eu
2. Wszelkie kwestie dotyczące danych osobowych można kierować na e-mail: iod@fundacjaukraina.eu, adres siedziby wskazany w ust. 1) lub tel. 571 330 203
3. Moje dane osobowe przetwarzane są/będą w celu realizacji projektu, w szczególności potwierdzenia kwalifikowalności wydatków, udzielenia wsparcia, monitoringu, ewaluacji, kontroli, audytu i sprawozdawczości oraz działań informacyjno-promocyjnych, a także w celach związanych z odzyskiwaniem środków, celach archiwalnych oraz statystycznych;
4. Przetwarzanie moich danych osobowych jest zgodne z prawem i spełnia warunki, o których mowa w art. 6 ust. 1 lit. b i c oraz art. 9 ust. 2 lit. a rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych) (Dz. Urz. UE L 119 z 04.05.2016, str.1), dalej zwane RODO.
5. Odbiorcami moich danych osobowych będą:
Partner – realizator projektu,
Beneficjent - Lider projektu, tj. Dolnośląski Urząd Wojewódzki z siedzibą przy ul. plac Powstańców Warszawy 1, 50-153 Wrocław,
Centrum Obsługi Projektów Europejskich Ministerstwa Spraw Wewnętrznych i Administracji z siedzibą przy ul. Puławskiej 99a, 02-595 Warszawa
Ministerstwo Spraw Wewnętrznych i Administracji z siedzibą przy ul. Stefana Batorego 5, 02-591 Warszawa
oraz podmioty, które na zlecenie Partnera – realizatora projektu i Beneficjenta uczestniczą w realizacji projektu. Dane osobowe mogą zostać przekazane w celu monitoringu, sprawozdawczości i audytu realizowanego projektu wyłącznie podmiotom uprawnionym do przeprowadzania powyższych czynności.
6. Moje dane osobowe będą przechowywane przez okres 6 lat od zatwierdzenia przez Organ Delegowany (tj. Centrum Obsługi Projektów Europejskich Ministerstwa Spraw Wewnętrznych i Administracji) raportu końcowego z realizacji Projektu;
7. Podanie danych jest warunkiem koniecznym otrzymania wsparcia, a odmowa ich podania jest równoznaczna z brakiem możliwości udzielenia wsparcia w ramach projektu
8. Posiadam prawo dostępu do treści do swoich danych oraz prawo do ich sprostowania, ograniczania przetwarzania, wniesienia sprzeciwu. W tym przypadku nie ma zastosowania prawo do usunięcia danych, gdyż przetwarzanie moich danych jest niezbędne do wywiązania się z prawnego obowiązku wymagającego przetwarzania na mocy prawa Unii Europejskiej oraz prawa państwa członkowskiego, któremu podlega Administrator
9. Mam prawo wniesienia skargi do Prezesa Urzędu Ochrony Danych, gdy uznam, iż przetwarzanie danych osobowych narusza przepisy RODO
