While the school is taking precautions to prevent the spread of COVID-19 on campus, I understand that my participant's involvement in athletics at the school and using our facilities may increase the risk that the participant could contract COVID-19. This means participants and participant's families and others coming into contact with participants could suffer damages caused by COVID-19 which can be extremely severe and include respiratory distress, organ damage, coma, and death.  I agree and understand that participation in school the athletic program at Mater Dei High School is entirely voluntary.  Knowing these risks, I on behalf of the participant, nonetheless knowingly agree to assume these risks.  I understand that if I or the participant have questions about possible COVID-19 hazards, it is your responsibility to seek additional information from the school staff before signing this agreement.    

I agree to comply with all additional rules and protocols adopted by Mater Dei High School regarding the use of facilities and equipment.
