Thank you for taking part in this survey, aimed at those who have served in the UK Armed Forces and have received a PTSD diagnosis, or those who believe they are suffering from PTSD. The responses remain anonymous and will be used to provide us with a broader understanding of the journey of those diagnosed with PTSD who are serving or have served in the UK Armed Forces.

The survey consists mostly of tick-box questions and should take less than 10 minutes to complete. Please remember that you can stop taking this survey at any point by simply closing your browser window, and you should do so if you become upset by it.
