All requests must be approved in advance; otherwise the absence will be recorded as unexcused. Each student is allotted 5 family education trip/college visit days per academic year. Once a student exceeds the 5 approveddays, all future absences of this kind will be unexcused.

Please print out and have your student have their teacher's sign this form to acknowledge they are aware of the absence. Form should be returned to the main office to Mr. Barth after it is complete.
