This form is to request replacement of Naloxone that has been used, has expired, or is otherwise no longer available.  This request is for Naloxone / Narcan only and not for the complete kit.  Please be advised that we cannot ship Naloxone / Narcan to you, but will contact you to arrange for you to pick it up if it is available.
