Please fill out this form for your submission for one of our four awards of excellence. For more information on our awards please visit our website at cnie-rcie.ca. If you are submitting multiple entries please use one form per entry.

The 2020 Deadline for Submission is 7 February 2020. Qualified parties may submit as many entries as they wish. A single submission can be considered for either an Award of Excellence or an Award of Merit in a single category. Each submission will only receive recognition in a single category.

Each entry must contain:
(1) A completed submission form with at least one member of the team being an active CNIE-RCIE member.
(2) Access to the resource submitted (preferred: URL with appropriate account and passwords if required), if your submission is not available online then the submission must contain the resource itself. If you do need to send your submission through USB stick or other format please ensure that you provide 5 sets of the resource.
