Medical Release/Liability Waiver

I hereby authorize the members of the Hoelck Skills Academy and coaching staff of Hoelck Skills Academy to act for my camp participant according to their best judgment in any emergency requiring medical attention. I hereby waive and release the camp’s personnel and associated staff from any liability for any injuries or illnesses (Including Covid-19 or related derivatives) incurred while attending the Hoelck Skills Academy.  The camp will not be responsible for lost or stolen items.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Hoelck Skills Academy during the selected camp. In exchange for the acceptance of said child’s candidacy by  Hoelck Skills Academy, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve, and hold harmless Hoelck Skills Academy and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions. In case of injury to said child, I hereby waive all claims against Hoelck Skills Academy including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including football. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death. 

Social Media Consent

I hereby authorize Hoelck Skills Academy to use my photo, video and/or information related to my experience. I understand this information may be used in publications, including electronic publication, audiovisual presentations, promotional literature, advertising, community presentation, media outlets and/or other similar ways. I understand that my participation is voluntary and I am free to withdraw at any time, without giving any reason.
