Year 6 & 7 Activities Week Consent Form
Year 6 and 7  will be heading to Yarrahapinni for Activities Week, November 28 November to Friday 2 December. We will be meeting at the front of Middle School at 8.45am on the Monday and will return by 3.30pm on the Friday, in time for normal dismissal.

Location: Yarrahapinni, 336 Grassy Head Road, Grassy Head, NSW 2441 
Cost: All costs/travel included as part of Activities Levy
Transport: Students will be transported to and from TAS via Oxley Explorer Coach.

This excursion includes planned water based (kayaking) activities. 

* Any student participating in a water activity (kayaking) will be provided with lifejackets and a safety briefing.

* Staff leading water activities have required First Aid/CPR/ Water Rescue qualifications. 

I consent to my child participating in water activities during the planned excursion (November 28 - December 2)

This excursion includes rope activities, including a challenge course, horizontal bungy and high ropes course. 

* Any student participating in a ropes activity will go through a full safety briefing and orientation. 

* Staff leading water activities have required qualifications. 

I consent to my child participating in rope activities during the planned excursion (November 28 - December 2)

This excursion includes swimming activities, including a waterslide.

* I understand that I am submitting this Consent form on behalf of my child/children or those students of The Armidale School for whom I am responsible

* By submitting this form, I declare that all information I have given is true and correct and that this form will serve as my signed permission for my child to attend the Year 6 and 7 Activities Week from Monday 28 November to Friday 2 December 2022. 

* If at any time during the excursion my child suffers injury or illness, I give consent for the School to arrange for medical treatment.  An authorised representative of the School is hereby empowered to give authority for such treatment

* I acknowledge that this event will be held in accordance with any current NSW Health COVID-19 Public Health Orders and in accordance with School policies and procedures. I acknowledge and accept that there is a risk that my child may be exposed to COVID-19 whilst attending and participating in school events. I confirm that my child will not attend if displaying any symptoms of illness, and/or is directed to be isolated under public health orders.
