CSI Event Registration Form
Please make sure to fill out the form completely and CSI will email you a confirmation for your event as soon as possible. Please remember that all events have to be registered at least five class days before the event date. All events will be uploaded to Guidebook once approved.

Events with alcohol need to be registered at least two weeks in advance. There is supplemental paperwork that needs to be completed and documentation that needs to be collected for alcohol registration events.
