COVID Screening: Abraham Erb YDP
This screening, combined with program attendance records where applicable, may be used to support contact tracing in consultation with Public Health.

This online screening must be completed daily for each child or staff who is scheduled to attend (including if they are not attending due to new symptoms). The form must be completed prior to entering the building (or before 8 am, whichever comes first). Visitors, including parents, who have been approved to enter the building are required to complete a COVID-19 screening as well.

SYMPTOM ASSESSMENT GUIDANCE:
When assessing for the symptoms below,  you should focus on evaluating if they are new, worsening, or different from your/your child’s baseline health status or usual state. If yes, select that symptom. Symptoms associated with known chronic health conditions or related to other known causes/conditions should not be considered unless new, different or worsening (If no, leave blank and select None of the Above). Note that if you or your child develop a cough or runny nose that you feel is 'just a cold' but it new, this would not be considered a chronic condition and must be evaluated by a health care professional or ruled out by a test.

SCREENING RESULTS & NEXT STEPS:
This screening has been updated to include new provincial direction.

If anyone in your household reports ANY SYMPTOMS  of COVID-19 (1 or more)  that are new or worsening   everyone in your household must stay home and isolate. The isolation period for the symptomatic individual can end when ANY of the following happen:
• They receive a negative COVID-19 test result
• They receive an alternative diagnosis by a health care professional, or
• It has been 10 days since their symptom onset and they are feeling better

 This isolation period for the asymptomatic family members (those with no symptoms) can end when ANY of the following happen:
• The negative test results are received and confirmed
• There is an alternate diagnosis from a medical professional
• 14 days have passed since the last contact with the symptomatic family member (if there is no test result or alternate medical diagnosis

Please know that we understand that this is a change from past practice.  Also, please know that this is the mandate of the Ministry and Public Health.
