Numerly Chapter Formation Form
Please fill out this form to start your own chapter of Numerly at your Local City!

Requirements:
- Must be able to connect with the local Independent School District
- Must be able to advertise and contribute to Numerly various courses/programs
- Must be able to recruit local tutors (volunteer tutors)
- Must be able to advertise to local students in your community/city
- Must be a High School Student (Grades 9-12)

Please check Numerly's website to make sure there is not an existing chapter at your local town/city. Ones approved, the person(s) filling out this form will become the Operational Head, and respected team for your chapter, unless stated otherwise.
