Electronic STD Treatment Reporting Form (VD91) for West Virginia
For use by providers when reporting STD treatment information to WV DHHR BPH Division of STD, HIV, Hepatitis, and Tuberculosis. All information that would be entered and submitted on a VD-91 card can be entered into this electronic form and submitted to STD Surveillance for faster processing.
