We are a group of researchers at University of Washington interested in hearing how COVID-19 has affected the operations at your worksite, and what concerns you have as your business may begin to re-open soon. All questions are optional. Your answers will be kept confidential, but we hope to aggregate responses across industries in order to share best practices , develop a guide that could inform subsequent public health or disaster events, and provide health and safety guidance to your worksite.

This study was reviewed by University of Washington Human Subjects Division and determined to be exempt from human subjects protections.
