This form is for you to nominate as an official to assist the Hockey Albury Wodonga Representative teams as a Coach or Assistant Coach.

By submitting this form you agree to abide by the Hockey Australia Code of Conduct and are also bound by the Hockey Albury Wodonga Representative Code of Conduct which is on the HAW website.  You are also committing to have a Working with Children check.

We currently need Coaches for the 2020 Junior Country Championships.

Selection will take place during the last 2 Academy sessions.
*    U13 and U15     Monday 2nd & 9th March 5:30 to 7:00
*    U18                    Thursday 5th & 12th March 5:30 to 7:00
You will be required to attend both these sessions.

The Competition is on the weekend of 16th & 17th of May in Melbourne
Under 13 Boys & Girls at Essendon Hockey Centre
Under 15 Boys & Girls at Greensbrough Hockey Centre
Under 18 Boys & Girls at Footscray Hockey Centre

This form must be completed and submitted to HAW before 26th February 2020.
