I represent that I am the legal guardian of the above registered child, and I hereby give Calvary Bible Church leaders and VBS staff the right and permission, with respect to photographs and video taken of my child, or in which my child may be included with others, to use, re-use, publish and re-publish the same in whole or in part, severally or in conjunction with other photographs, in any medium and for any purpose whatsoever including illustration, promotion and advertising, and trade (excluding anything illegal or immoral). I also give my permission for the Calvary Bible Church leaders and VBS staff to administer First Aid as needed (cuts, scrapes, etc.) and to seek further necessary medical attention if necessary (hospital, ER, etc). I hold the Calvary Bible Church leaders and VBS staff harmless and not liable in the exercise and use of this authority, and I accept responsibility for any and all costs that should come from any medical services. I understand that a conscientious effort will be made to notify me before such actions are taken, but if I am unable to be reached the Calvary Bible Church leaders and VBS staff will move forward with the necessary medical treatment for the safety and well-being of my child.  I understand that my electronic signature is equivalent to a handwritten signature, and thus constitutes a legally binding document.
