By attending a KBJ Academy skills training, you understand that Kevin and Brenita Jackson will be instructing basketball specific drills for all athletes in attendance.  Athletes are able participate in drills they choose and accept full liability for participating in drills.   By completing this form, you agree to release Kevin & Brenita Jackson and KBJ Academy from any form of litigation now and forever.  You agree to allow us to take pictures and videos for promotion purposes and release the use of the media files.  All workouts are optional and student-athletes should be cleared by a physician prior to work outs.  Payment for the skills session is due prior to attending the training session.  Prior to attending, your athlete will need to disclose if he/she has experienced any symptoms related to COVID. Student athletes with symptoms will not be admitted if you have been in contact with anyone with a lab confirmed COVID test or if you have been confirmed with COVID and needs to quarantine for 14 days.  As the parent/legal guardian of your student-athlete, please type your first and last name below to confirm your consent to these statements. *
