Polo & Shirt Order Form - Blackman Middle School Band 20-21

Your student will need to have BOTH a black BMS Band polo (for concert attire) and a navy BMS Band shirt (for pep band, trips, pictures, spirit days, etc.) to use in band.

Please choose from the following sizes:

Youth Large, Small, Medium, Large, X-Large, 2X-Large, or 3X-Large

*All sizes are ADULT except for Youth Large

*T-Shirts are $8.00, Concert Polos are $15.00

*If you are ordering one of each shirt in regular sizes, you would owe $23

Please send payment to the band room for the shirts ASAP!
