This is an informal tracking of COVID cases at Marshall Elementary School. This form is completely voluntary and anonymous. It is meant to bridge the gap from the cases not reported by SOMSD and report those cases from in-home rapid tests and show which classes have been exposed. Please fill out this form for each individual child affected.
