Please note that once you are able to submit your information through this form and are then contacted to schedule, pre-payment for surgery will be required from all clients to hold your surgery spot.

This will only be our second price increase in 21 years of operation. We are committed to continuing to provide high quality services despite the challenges of COVID-19 and increase in operations cost. We appreciate your understanding!
