Liability Waiver Form
You, the parent or guardian, hereby acknowledge and agree to the following terms and conditions in consideration of my child/children listed above, being permitted to participate in the Pom and Jazz Camp (hereinafter referred to as "the Camp"):
1. Assumption of Risk: I understand that participation in the Camp involves certain inherent risks, including but not limited to, minor injuries, sprains, strains, and more serious injuries such as broken bones or concussion. I acknowledge that I am voluntarily allowing my child to participate in the Camp activities with full knowledge of the risks involved.
2. Release of Liability: In consideration of my child being permitted to participate in the Camp, I, on behalf of myself, my child, and our respective heirs, successors, and assigns, hereby release, waive, discharge, and covenant not to sue U Will Dance Studio, Griset Damas Roche or Jasmine Brown, its owners, employees, agents, representatives, volunteers, or any affiliated entities (hereinafter collectively referred to as "the Released Parties"), from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or any property belonging to my child while participating in the Camp activities, regardless of whether such loss, damage, or injury is caused by the negligence of the Released Parties or otherwise.
3. Medical Treatment Authorization: I authorize the Camp staff to seek medical treatment for my child in the event of an emergency if I am unable to be reached. I understand that every effort will be made to contact me or the emergency contact listed below prior to any medical treatment being administered.
4. Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, liabilities, damages, losses, costs, expenses, or fees (including reasonable attorneys' fees) arising out of or related to my child's participation in the Camp activities or any breach of this agreement by me or my child.
I have carefully read and fully understand the contents of this Liability Waiver Form. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue. I voluntarily agree to be bound by its terms.
U Will Dance and its teachers reserves the right to refuse participation to any individual who does not comply with the rules and regulations of the Camp or poses a risk to themselves or others.
