1. AGREEMENT
You acknowledge that this agreement takes effect at the time of you submitting this enrolment form.

2.FEES
Fees are due for payment before or on the day of the first lesson via cash, eftpos or direct debit. Payment plans are available for invoices over $300 per term. In the event of unpaid or overdue fees, we reserve the right to engage a Debt Collection Agency. You will be liable to pay for any collection agency fees or administration fees occurred.

3. REFUNDS
It is a policy of EBA that in the case of a student withdrawing from a course after the term has commenced, refunds WILL NOT be made. In the case of absenteeism in class, you are mot welcome to complete a missed session at a later time within the term that the lesson is missed. Refunds WILL NOT be made for lessons missed or a change of mind.

4.UNIFORM & GROOMING
EBA has a compulsory uniform that students are required to wear each week. There is also no loose jewellery to be worn, and hair must be in a bun for ballet and a slick ponytail for all other styles. No uniform = No class.

5.CHILD COLLECTION
It is the parent's responsibility to collect students immediately after class. There is not supervision in the waiting room.

6. PHYSICAL CONTACT
Parent's are advised that some physical contact will be necessary by the teacher at times to assist with demonstration of exercises or techniques, or if a staff member needs to administer first aid.

7. WORKING WITH CHILDREN
All EBA staff are required to hold a valid Working With Children's Check

8. COMMUNICATION
All communication may be done via phone (0410500211) or email (erin@ellenbrookballet.com) or in person at our reception desk. Teachers are not to be contacted directly or personally.

8. FIRST AID
In the event of an emergency and parents are not able to be contacted, it is understood that the teacher reserves the right to call an ambulance service.

9. CANCELLATION OF ENROLMENT
EBA reserves the right to cancel your enrolment at any time when any of the following occurs;
a) Unpaid invoices.
b) Not adhering to the above T&Cs.
c)You fail to comply with any reasonable direction issued by a staff member that compromises the safety and wellbeing of our staff & students.
d)If in our sole discretion we consider it is no longer appropriate to provide our services to you.

10. SUSPENSION OF IN PERSON CLASSES
In the event of a Government imposed closure all classes will be immediately taken online via zoom. There will be no change in fees and no refunds. You fully understand and accept this risk when enrolling that a closure out of our control may occur at any time, and in doing so agree to attending live zoom classes instead of in-person classes and will not be refunded. If you are not able to/comfortable with attending live zoom classes you will be able to be sent a pre-recorded class. No credits will be given for when we re-open again.

11. APPLICABLE LAW
The laws of Western Australia apply to this agreement and any Commonwealth law that may have jurisdiction.
