Thank you for choosing to vend during our 15th Chapter Anniversary & Scholarship Awards Gala.

PLEASE NOTE:
• Use one form per vendor.
• Space is limited.
• ALL wares to be sold must be approved.  If you have questions, please ask BEFORE you submit your form. Send questions to: foundation@phialphazeta.org.
• Reservation will be confirmed once payment is received. Once confirmed, a contract will follow.
• ALL vendor requests and payments MUST be received no later than FRIDAY, March 4, 2022.
• Proceeds support the scholarship program, thus no refunds will be given.
