My parents and I have reviewed the information about the Mission Trip Program and have discussed my participation.  I intend to participate in the activities with an open mind and generous heart.  I understand that:
• the program culminates, not ends, in spending a week in either Kentucky doing service with those in need.
• the program includes many activities ranging from local service projects to formation meetings to fundraisers in which I must participate as outlined in the information for the program. I will regularly check the Holy Spirit youth ministry website (https://hsyouthunoh.weebly.com/) for the latest information about the program.
• participation in the program and specific events requires compliance with a code of conduct and I agree to abide by it.  Infractions may result in my dismissal from the program.
