This form is for applying to be on the Electoral Roll (Membership List) of Berwick Anglican Church (the Parish of Christ Church Berwick).

You must be on the Electoral Roll in order to vote at the annual meeting and to nominate to be an office bearer at Berwick Anglican Church.

The criteria is that all answers on this form must be answered with a YES. If any answer is a NO, you cannot be a member of the electoral roll at Berwick Anglican Church.
