In an effort to alleviate the dismissal confusion on UNSCHEDULED early school closings (days not on the school calendar), we ask you to complete the following form. Please be very clear and specific as this is the only information teachers and staff will reference on Emergency Early Dismissal days. There is no need to call the school. Please complete a SEPARATE FORM for each child.

Please choose ONLY ONE of the following dismissal options (the most likely to occur). IMPORTANT NOTE: The BAS Program will NOT be open after school on EMERGENCY early dismissals, so please choose another option for dismissal! *

My child should take their regular bus home.

My child should take their regular bus to their DAYCARE provider (please list name of daycare provider below).

Alternate address on your child's bus route (provide name and alternate address below).

My child will be picked up (list pick up person's name below). If this is someone other than a parent, please make sure they bring ID with them to show before picking up your child.

Please provide additional information requested in previous question (i.e. name of daycare provider, NAME and address of alternate address on your child's bus route, or name of person who will pick up your child on an Emergency Early Dismissal day)

I understand that BAS will not be open if there is an Emergency Early Dismissal.

I understand that notification of an Emergency Early Dismissal will be provided via the telephone, text and/or email I have provided in the School Messenger app.

I understand that my child is not allowed to change buses, so cannot go home on a different bus.

I understand that if I do not complete this form, my child will be sent home on his/her regular bus on an Emergency Early Dismissal day.
