This waiver is required for all children who will participate or are participating in school-sponsored athletic programs in the Diocese of Covington. One submission/acceptance per year per athlete is required and will cover participation in all Diocesan-sponsored sports. No child may participate in school-sponsored athletics unless this form has been accepted and submitted and/or submitted on paper to the school.

Waiver & Release for Communicable Diseases Including COVID-19

In consideration of my child participating in a school-sponsored athletic program, the undersigned acknowledges and agrees as follows:

1. My child’s attendance and participation in the school-sponsored athletic program is voluntary, and my child and I are aware of the COVID-19 virus and its risks as reported by the Centers for Disease Control and Prevention (“CDC”);

2. I understand that my child’s attendance and participation in the program includes possible exposure to and illness from infectious diseases, including MRSA, influenza, and COVID-19, and the risk of serious illness, disability, and death from those infectious diseases does exist;

3. I knowingly and freely assume all such risks, both known and unknown, and assume full responsibility for my child’s participation in the school-sponsored athletic program;

4. I willingly agree to comply with the stated and customary terms and conditions for participation in the school-sponsored athletic program and will take the necessary steps to protect against infectious diseases, including abiding by all rules to keep my child, other children, and school employees and volunteers safe;

5. I, for myself and on behalf of my child, spouse and our heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless my child’s school, the Roman Catholic Diocese of Covington and their officers, board members, officials, agents, and/or employees, and other participants, sponsors, advertisers, and if applicable, owners and lessors of the premises used to conduct the event (“Releasees”), from and against any and all claims, counterclaims, demands, debts, obligations, liabilities, losses, causes of action, or lawsuits of any kind and nature, whether known or unknown, in any way related to my child’s participation in the school-sponsored sports program.

6. I, for myself and on behalf of my child, spouse and our heirs, assigns, personal representatives, and next of kin, hereby acknowledge that all of the potential long-term health impacts of COVID-19 are not known at the present time and agree to release the Releasees and hold them harmless in the event that my child suffers any COVID-19 related health issue in the future.
