We're so thrilled to welcome you and your student to the 2024 All-City Middle School Musical: The SpongeBob Musical Youth Edition!
Whether your student is interested in performing onstage or designing, building, and running backstage, we know this will be an opportunity for them to discover new potential and be a part of a wonderful, inclusive theatre community while they learn.
This form is intended to collect important information that will help us ensure students have a safe and positive experience as a part of the All-City Middle School Musical. It should take between 5-10 minutes to complete. Before beginning this form, it would help to have the following pieces of information ready to go:
- Your student's grade, school, and any allergies or medical notes that we should be aware of
- A contact phone number and email for one Family Member
- A contact phone number and email (optional) for one Emergency Contact
- Any specific dates that your student will not be available for rehearsals or performances (i.e. appointments, vacations, other school or community commitments).

This form must be filled out in order for your student to participate in The SpongeBob Musical. Please fill it out no later than Friday, February 16.
