If a student is checking out a student device during the 2020-2021 school year, please fill out this form for each student individually.
This form should be filled out online before coming to the checkout station at a school or the TSD Support Services Center, 2890 N Monroe Ave, Loveland, CO 80538. Please do not come to the checkout station if anyone in your family is exhibiting symptoms of illness.
Parents/Guardians must:
1. Make an appointment to check out a device by selecting a time slot at

2. Have registered the student for school including indicating their understanding of the Student Acceptable Use Policy and Agreement;
3. Complete this form before coming to check out a device.
