Parent or Guardian must complete and submit a form that gives consent for participation, photo release, and medical information. The form is found on the Metro Nashville Chorus YWISH web page. The form can be mailed or scanned/emailed. Forms must be received by 10/1/24

Registration Payment Option *

YWISH registration is $25. Your Parent/Guardian can submit a check via mail OR he/she may pay online (via PayPal) on the Metro Nashville Chorus YWISH web page. Payment must be received by 10/1/24.
