GJYS & GYS/IBOC Summer Program Registration Form 2021
     The summer program is from June 7 to July 2, every week-day from 8:00am to 9:30am for Sr. & Jr. Symphony and, from 9:30am to 11:00am for iBOC, at Cottonwood High School, 5175 South 1300 East, Murray.

All students are required to participate in our "Test to Play" COVID screening.  On June 7th, 2021 students must provide proof of a recent (no more than three days prior to June 7, 2021) negative COVID test or receive a "Rapid COVID test on site, free of charge.  Any student who tests positive must be picked up by a parent/responsible individual.  We are unable to transport students who have tested positive for COVID.  (An initial positive COVID test will also exclude the student from participating in the Youth Symphony camp and any funds will be refunded).  If a student tests positive on the second Rapid Test, to be administered on site, Monday, June 14th.  No fees will be refunded.  Please acknowledge that you have read and agree to the above terms. *

Do you require transportation?  Buses will pick up students at selected junior high schools, TBD (to be determined). *

We will provide a free sack lunch to interested IBOC students.  These lunches will be available at 11:00 am after the IBOC session and before students depart on buses.  Please let us know your preference. *
