Absences and tardies must be "cleared" using this form or by calling the Attendance Office within 48 hours of the absence.  
Absences that are not cleared within 48 hours will be turned into truancies. Students should monitor
attendance online and talk to their teacher if corrections need to be made.

For early dismissals, students will need to pick up a Permit-to-Leave at the attendance office so the teacher can release the student on time. A parent/guardian will need to notify the attendance office via this form or via phone call before a permit can be given to the student. (Failing to do both BEFORE the student leaves campus will result in a truancy)

NOTE ABOUT EXTENDED ABSENCES DUE TO ILLNESS: A medical note must be provided when a student has had 5 or more consecutive absences due to illness. When a student has had 14 absences in a school year for illness verified by a parent/guardian, any further absences must be verified by a physician and a medical note provided to the attendance office.

IMPORTANT NOTE ABOUT EXCUSED AND UNEXCUSED ABSENCES: According to California Education Code, the following are considered excused absences: illness, medical appointment, religious holiday, family emergency, jury duty or funeral services. Reasons for unexcused absences include: being out of town, vacation, college visits, and non-school related activities.

We are using this form in order to support families in reporting their student's absences in an easier way, and to save staff time that can be shifted to supporting students with serious absence issues.

POR FAVOR TENGA EN CUENTA: Todos los formularios de ausencia deben presentarse dentro de las 48 horas, usted está aclarando una ausencia dentro de las 48 horas (2 días) al regresar a la escuela. Las ausencias sin un formulario enviado dentro de este tiempo se convertirán en ausencias injustificadas.

Si su estudiante tiene que salir temprano de la escuela, tendra que recoger un pase en la oficina de asistecia. Ud. tendra que llenar este formulario o reportar la salida por telefono antes de que su estudiante reciba el permiso para salir temprano. No hacer ambas cosas ANTES de que el estudiante salga del la escuela resultará en absentismo no justificado.

NOTA SOBRE AUSENCIAS EXTENDIDAS DEBIDO A ENFERMEDADES: Una nota medica sera necesaria si su estudiante falta 5 dias o mas de escuela por razones de enfermedad. Cuando un estudiante ha tenido 14 ausencias en un año escolar por enfermedad verificada por un padre / tutor, cualquier ausencia adicional debe ser verificada por un médico y una nota médica entregada a la oficina de asistencia. Si ud. ha otorgado una forma de "Enfermedad crónica" durante éste año escolar, ud. siempre tiene que verificar cada  ausencia.

NOTA IMPORTANTE SOBRE AUSENCIAS JUSTIFICADAS Y NO JUSTIFICADAS:  De acuerdo con el Código de Educación de California, las siguientes ausencias se consideran justificadas: enfermedad, cita médica, dia religioso, emergencia familiar, servicio como jurado o servicios funerarios(a familia inmediata) . Las razones de las ausencias injustificadas incluyen: estar fuera de la ciudad, vacaciones, visitas a colegios y actividades no relacionadas con la escuela.

Estamos utilizando éste formulario para ayudar a las familias a informar las ausencias de sus estudiantes de una manera más fácil, y para ahorrar tiempo al personal para poder apoyar a los estudiantes con problemas graves de ausencia.
