All guests are required to check in and sign our Confidentiality & Waiver Agreement to protect the confidentiality of the students and clients at Sunshine School & Development Center in accordance with State and Federal Regulations.

To protect the confidentiality of students at Sunshine School & Development Center in accordance with State and Federal Regulations any records of the identity, diagnosis, prognosis, or treatment of any student which are maintained in connection with the performance of any program at the Sunshine School & Development Center shall be confidentiality in conjunction with state law. Under Federal Regulations you may not disclose any information regarding students at the Sunshine School & Development Center.

I understand that this Waiver discharges Sunshine School & Development Center from any liability or claim that I, the Volunteer, may have against Sunshine School & Development Center with respect to bodily injury, personal injury, illness, death, or property damage that may result from my participation on Sunshine School & Development Center's premises. I also fully understand that Sunshine School & Development Center does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage.

I represent and certify that my true age is 18 years or over, I certify that my attendance and participation on the premises is voluntary and that I am not, in any way, the employee or agent of the owner, operators, of the premises and the activities, vehicles and equipment. Sunshine School & Development Center also has my permission to use any photographs or video footage taken during volunteer activities in any and all publicity.
