Consent Form for Emergency Home Delivery Meals During School Closure - COVID 19/NTI.

The Kentucky Department of Education School & Community Nutrition Division is allowing home delivery of meals during the school closure as a result of the COVID-19 virus. Schools must obtained a signed consent form from a parent/guardian in order to make home deliveries.

Not only do you need to complete this survey - please also call the district office to confirm receipt of your information.
