1: I understand that a permission number will not be issued until I have met the prerequisite for the course or have permission from the Chair of the Math Department. Prerequisites must be posted to your eCampus account (for example: transfer credits, AP Calc scores) or your exam score in the URI Math Placement system. When the prerequisite is listed in eCampus, please email bkaskosz@uri.edu to confirm.
2: I understand that the Department of Mathematics and Applied Mathematical Sciences of the University of Rhode Island will use the email address provided on this form as the sole means of contacting me, and I will check that email address regularly.
3: I understand that if I am given a Permission Number to enroll in a course, I will have until midnight, the day following issuance, to use that Permission Number.
4: I understand that if I do not use my Permission Number by midnight, the day following issuance, I will be dropped to the end of the Permission Number list.
5: I understand that if I already have a section assignment, and if I then apply for a new section, I will be considered only after all applicants who are awaiting acceptance into the new section have been processed.
6: I understand that no Permission Number will be issued to me if I have any Hold on my account.
7: I understand that no Permission Number will be issued to me if the course is full.
8: I understand that no Permission Number will be issued to me if I select "I DISAGREE" below.
9: I understand that the submission of a permission request does not guarantee the issuance of a permission number.
