SMPO Read-a-Thon reading log
This form should be completed by a parent or guardian, and not a student. Please complete the form for each student reader with the total minutes read during the 2022 SMPO Read-a-Thon. The information submitted here will be used by the Read-a-Thon committee to finalize your student's fundraising page to process any pledges per-minute, as well as track prize eligibility.

It is vital that Read-a-Thon information is verified by a parent before submitting and that a parent completes this form for their student. This helps ensure accuracy and fairness to all participants.
