After you complete this form, you still need to sign the Hold Harmless Waiver, and submit payment for your plot. You can mail a check, or pay cash to Program Manager Susan Oddsen in person. Checks should be made out to La Crescent Community Garden.  Mailing address: La Crescent Community Garden, 333 Main St., La Crescent, MN 55947. There is potential that we will be able to set up an online payment for a small processing fee, more information will be coming.
