Prior to submitting this form, you will need the following:

• Review the information on the Graduate Division website:

• To establish your ASSIST identifier: create an eRA Commons user ID (https://grad.ucsd.edu/financial/fellowships/funding-opportunties/NIH%20F31.html#How-to-Apply) and begin an application in ASSIST (https://public.era.nih.gov/assist). Your application will be assigned a six-digit number, this is your ASSIST identifier (example 813456)

• To have your fund manager's contact information: contact your advisor/mentor and ask them for the name and email of their fund manager
