Giving Project Application to Register
Hello and thank you for your interest in Chinook Fund's Giving Project! It will take approximately 30 minutes to fill out this Application to Register form.

This form includes 5 short parts:
1) Your contact information,
2) How you were referred to us
3) Some logistical information
4) Your demographic information
5) A pre-survey to give us a sense of your background and skills
   (FYI: There is no minimum or maximum level of expertise required to join a project! We will use this information to better prepare for each session.)
6) Space to share a little more about yourself

NOTE: This is a multi-page survey! Please click 'Continue' at the bottom of the page to completely the next section. Also, this form does not save your responses before you submit them: if you close the tab, you cannot return to complete the form. We highly recommend writing your answers in a Word or Google document and copy-and-pasting them into the form.

After you finish this survey, our team will be in touch! Each cycle, we consider the diversity of our cohort across race, class, and other dimensions of identity. If we have heavy representation in one area, we may ask an applicant to remain on a wait-list, serve as an alternate, or defer their application for the next Giving Project cycle.
