TRIP REQUIREMENTS

  1. Attend prayer meeting(s) determined by trip leader. (Prayer Meetings are each Tuesday in Osborn Room from 6:00-7:00 pm)                                                                                                                                                           
  2. Attend team meetings. (Time and date provided by leader)                                                                  
  3. Obtain proper equipment for trip.  (Packing list provided by trip leader)
  4. In-country flight. State ID / Driver’s license, Birth Certificate for children
  5. Complete Medical & Health information sheet for each family member traveling. (Will be given after this application is filled out)                                                                                                                                                 
  6. Complete Liability & Release Agreement for each family member travelling. (Will be given after this application is filled out)
