On-Site Attendance Application Form

For Term 4 2021, the Premier has announced new restrictions and a staggered return for students to access onsite learning.
From Monday 4th October to Thursday 4th November all Victorian Metropolitan schools will continue a period of remote and flexible learning.
From Monday 18th October, there will be a staggered return to school for all students, with everyone returning on Friday 5th November.

This Google Form has been developed based on the the revised and currently available criteria. THIS FORM WAS LAST UPDATED 21st October, 2021.

The Department of Education and Training has determined that this process is to be completed weekly to ensure adequate staffing on-site.  This form is for on-site requests for days students are timetabled to work remotely during the staggered return, from Monday 18th October to Thursday 4th November 2021 inclusive. Bookings must be submitted by 9am on Friday 22nd October 2021.

As with other remote learning periods, our school will provide on-site supervision for students in the following categories:
Children where both parents and/or carers are authorised workers who cannot work from home, work for an essential provider and where no other supervision arrangements can be made:
o Where there are two parents/carers, both must be authorised workers, working outside the home in order for their children to be eligible for on-site provision
o For single parents/carers, the authorised worker must be working outside the home in order for their children to be eligible for on-site provision.
o     Where no other arrangements for supervision can be made

An authorised worker permit is required for parents requesting on-site supervision for their child/ren. Authorised worker permits for ALL ADULTS in the home MUST BE PROVIDED TO THE SCHOOL, with times and dates corresponding to school booking times and dates.

Children experiencing vulnerability, including:

o children in out-of-home care
o children deemed vulnerable by a government agency, funded family or family violence service, and is assessed as requiring education and care outside the family home
o children identified by a school or early childhood service as vulnerable, including via referral from a government agency, or funded family or family violence service, homeless or youth justice service or mental health or other health service.
o     where an identified student with a disability is deemed vulnerable because they cannot learn from home

All eligible students will be cared for and supervised at school from 9:00am - 3:30pm daily. Please note - students on-site will be accessing the remote learning platform and will not be receiving direct instruction, as Casual Relief Teachers (CRT) and some ARPS teachers will be rostered on. Your child's regular classroom teacher or aide will most likely not be available.  

Students not attending before school care must not arrive prior to the 8:45am  for a 9:00am start to the day. Students will enter via Gate 2 ONLY (Albany Drive). Later arrivals must enter via the office area. The office will be locked so please phone 9547 1146 to gain entry.

Regular before and after school arrangements with OSHClub are in place and you are able to make a booking with them via https://www.oshclub.com.au/ for before and after school care.
