Lake Forest Middle School:  Revolution Kids Show Choir Camp Registration Form
PENDING APPROVAL TO RETURN TO CAMPUS DUE TO COVID19: All upcoming 2nd, 3rd, 4th, 5th, and 6th grade students from all County and City schools are invited to join the directors of the award winning Lake Forest Middle School show choir, Revolution, for 4 fun filled days of song and dance!  The Kids’ Show Choir will learn music and choreography to a variety of favorites and will present their show on Friday evening, July 17, 2020 to family and friends at 7:00 pm the LFMS Auditorium.

To register, please complete the information form below by June 8, 2020. (If submitting registration after this date, please email Mr. Pritchard at jpritchard@bradleyschools.org). The cost is $60 total (includes a T-shirt, rehearsal CD and music.) This fee is due the first day of camp, July 14.

Due to COVID19 uncertainties, please do not mail in any payments.
