EVENT Ticket Purchase
1.  Event tickets may be purchased by completing this form and submitting a payment of Venmo or check in the amount of $85, per ticket.  Ticket includes a full sit-down dinner and non-alcoholic beverages. (There is a cash bar.)

2. If paying by check, please make it payable to "SHSW PTSA"     Please include either an email or cell phone number in the memo of the check.  Check payments can be dropped in the  PTSA Mailbox located in the Main Office at Smithtown West.

3.  ***VENMO USERS***Very important!!!   If making a Venmo payment, please venmo@shswptsa    You should see the name Debbie Bresnahan...she is our treasurer.  To make sure these transactions are processed as personal payments, without fees or tax ramifications,  the "purchases" button should be in the "OFF” position, where it will appear as a Gray Circle.

4. Tables will be seated with a maximum of 10-12  people. If you would like to be seated together with family or friends, please submit ONE PAYMENT for the group and then list the individual names on the line provided on the google form.

5. We will confirm your ticket order via email. Physical tickets WILL NOT be printed. Please look for table assignments posted at the Fashion Show.

6. Tickets are NOT available at the door. Please purchase your tickets by February 21st, 2024.

7. COMING SOON...Raffle basket tickets will be available for pre-purchase through a separate form and will also be available for purchase the night of the event.
